  Your best bet is to contact the Library of Congress, and request
the proper forms. They can be reached at:

  The Forms and Publications hotline: 202-707-9100, will allow you to
directly order whichever forms you might need. Also, you can speak
with an information specialist by dialing: 202-707-3000.
  I think the forms you'll want are Forms 44, 40a and Form VA. It
used to cost twenty bucks to register a copyright, but that may have
changed over the years. Contact the Library of Congress for updated
pricing.
