  I think the main one was "Form VA", although there were numerous
informational brochures that came with the VA forms. These included
Circular 40 (Copyright registration for works of the visual arts),
Circular 40a (Deposit requirements for registration of claims to
copyright in visual arts material), Circular 1 (Copyright basics),
Circular 44 (Cartoons and comic strips), and whatever other forms
they may include. Bear in mind, these forms I'm quoting are from the
1995 registration of the Caterwaul series, so it's possible there
have been changes since that time regarding form requirements.

  To anyone interested in getting their work copyrighted, I would
suggest doing this, since the "poor man's copyright" won't really
hold water should you need to go to court over a conflict.
