Unfortunately, I have a story that tops that one. A since-retired man at my workplace told us of how he was at a hospital where an older patient was getting exposures from an X-ray machine that was defective. When the operator took the exposure, the machine produced excessive X-rays which burned the patient. And then the operator took more exposures which burned the patient further! D:
